Did you and your manager discuss your plans to take time-off to go to school or enroll in an education/training program and have you resolved any scheduling-related issues?

By proceeding to the application, you affirm that you have discussed your plans with your manager and have received approval to reduce time from your regular schedule. Your manager will be sent notification of this application upon submittal.
